If you are reading this while pregnant and still smoking, please know this first: you are not a bad parent. Nicotine is one of the most addictive substances on earth. Asking someone to stop using it overnight, during one of the most stressful periods of their life, is genuinely hard.
It is also one of the most important things you can do for your baby. We can help you do it, with kindness and without lectures.
Why it matters so much in pregnancy
Smoking during pregnancy is linked to a higher risk of:
- Miscarriage and stillbirth
- Premature birth
- Low birth weight (and the lifetime health risks that come with it)
- Sudden infant death syndrome (SIDS)
- Lung problems and asthma in childhood
The reasons are mostly carbon monoxide and the toxic chemicals in cigarette smoke crossing the placenta. Every cigarette reduces the oxygen your baby gets, even briefly.
Here is the good news: the benefits of stopping at any point in pregnancy are real and immediate. Stopping in the first trimester is best. Stopping in the second is still excellent. Stopping in the third is still genuinely worthwhile.
What about NRT while pregnant?
This is the question we get most. The short answer: NRT is far safer than smoking during pregnancy, and it is widely recommended as part of a stop-smoking plan if needed.
The reason is simple: a cigarette delivers nicotine plus carbon monoxide plus 4,000+ other chemicals. NRT delivers a controlled, measured dose of nicotine alone. For your baby, the difference is significant.
Your advisor will go through the options carefully and will usually start with the lowest dose that works for you. Many people use a 16-hour patch (removed at night) or an oral form.
What about vaping in pregnancy?
At our clinic, we support pregnant clients with traditional NRT — not vapes, in line with NHS guidance. Vaping during pregnancy is much less harmful than smoking, so if you have already switched to a vape, that is a real step forward — continuing your vape is far better than going back to cigarettes. But for clients starting their quit with us while pregnant, we'll work with you to build a plan around NRT rather than vapes.
The bigger picture: stress, partners, and shame
Often the reason people struggle to stop in pregnancy is that the people around them are still smoking. A smoking partner roughly doubles your chance of relapse. If your partner can join you — even informally, even if they do not see themselves as wanting to "quit quit" — your chances skyrocket.
And shame is a trigger. The more guilty you feel, the more you crave. Telling someone non-judgemental is often the first turning point.
