Shisha is social, relaxed and woven into many evenings out across Birmingham — and because the smoke passes through water and tastes of apple or mint, it’s easy to assume it’s the gentle option. What the water doesn’t do
The water bowl cools the smoke, which makes it smoother to inhale — but it does not filter out the harmful part. Tar, heavy metals and cancer-causing chemicals pass through with the smoke. Cooler doesn’t mean cleaner; it mostly means you can comfortably inhale more of it, for longer.
The session effect
A cigarette lasts a few minutes. A shisha session lasts an hour or more, with long, deep draws throughout. Health bodies including the WHO have warned that over a typical session you can inhale many times more smoke than you would from a single cigarette. And because the tobacco is heated by burning charcoal, shisha smoke carries especially high levels of carbon monoxide — the gas that crowds oxygen out of your blood. It’s why some regular shisha users fail the same breath test heavy cigarette smokers fail.
“But I don’t smoke cigarettes”
Shisha tobacco still delivers nicotine, so regular sessions can quietly build the same dependence — and sharing a mouthpiece adds its own risks. Herbal or “tobacco-free” mixes still produce smoke and carbon monoxide from the charcoal, so they’re not the loophole they appear to be.
